Hospice care is a choice when you and your physician determine that efforts to cure your illness are no longer feasible, or desirable. Our focus is to make sure you are as comfortable and pain-free as possible, and to help you make the most of every moment of living.
Our hospice care services go beyond relieving pain, answering questions and providing resources. We become our patients’ trusted companion and we customize the care experience to meet the unique needs and wants. Our focus is not on the illness, rather embracing experiences that bring meaning and joy to a person’s life.
Typically, a family member serves as the primary caregiver and, when appropriate, helps make decisions for the patient. Members of our hospice staff make regular visits to assess the patient and provide additional care or other services. Our hospice staff is on-call 24 hours a day, 7 days a week, and 365 days per year.
The patient and their hospice care team will develop a plan of care that meets each patient’s individual needs for pain management and symptom control. Our team usually consists of:
The patient and their caregivers
The patient’s personal physician
Hospice physician (or medical director)
Nurses
Home health aides
Social workers
Chaplains
Trained volunteers
Speech, physical, and occupational therapists, if needed.
"Each of the care team members provided by Tapestry Hospice were very responsive, proactive, and knowledgeable in every phase and aspect of our mothers care. They truly went above and beyond to ensure that our needs were met. When we needed them most they truly were there. Not sure what to say about the dying process, it’s never easy whether it is quick or slow…Either way it is easier with someone that knows what to expect. Tapestry Hospice was there for our family when my mom got her final cancer diagnosis. Knowing it was a matter of days maybe a week…they stepped in and helped us through the hardest patch of life…. leaving it. The nurses and chaplains and other staff were available to answer questions on just hold a hand and listen. Thank you for your commitment to my family. It is amazing how sharing your stress and sadness makes it a little easier to handle. Our family will be eternally grateful for your company and what they did for us. Kim A.

I had two experiences with Tapestry Hospice. Both were nothing less than exceptional! My parents were together in an assisted care facility for a total of 11 years. My mother, Monte Jean, suffered a lengthy illness in 2019. At the appropriate time, Tapestry stepped in to administer the final care she required before her passing. I don't know what I would have done without their compassion and quality of care. She passed at 95 years old. My father, Vernon, subsequently passed in 2023 at 102 years old. Again, Tapestry stepped in to ensure his end-of-days care was as dignified andcomfortable as possible. The nurses and staff treated us just like their own family. I cannot imagine better care than what Tapestry Hospice provided. I will be forever grateful and indebted to this wonderful organization. They tremendously helped my family through the stages associated with the final care oftwo people who were so deeply loved. Jana G.

Hospice does not mean you are giving up hope. It can provide a more thorough and refined layer of care that is personalized and specific to you or your loved one’s needs. Tapestry is here to help determine if you are appropriate for hospice care and to talk about options. Here are a few signs that may indicate it’s time to start the hospice conversation:
